Your First Visit

Understanding more about your medical history and where you are in your journey is essential to developing a treatment plan together. To better inform our recommendations, we collect this information through two simple steps:

1. New patient forms: Complete these forms via the Prelude Connect app at least 48 hours before your appointment.
2. Electronic release form: By signing this form, you authorize us to access relevant notes and records from other healthcare providers, enabling us to provide comprehensive and tailored care.

During the initial virtual consultation, your physician will review your medical history, discuss your family building goals and together decide on a treatment plan.

After the virtual consult, you will complete a transvaginal ultrasound and this will be your first visit to the office. During the ultrasound, our physician will access your antral follicle count and uterine lining, which will help finalize a treatment plan.
